Instructions for General or Committee Building Use E-Forms:

  • Click on the link for the specific form (Committee or General Use),
  • Download the form to your computer (ex. desktop or download folder), 
  • Open the form (ex. using Acrobat Reader) and type the information into the form,
  • Choose ‘File – Save As’ to give your form a unique name and save it to your desktop,
  • Email the pdf document to Julie Thompson (juliethompson@fpcp.net).
